In asking that my child becomes a full member of the Catholic Church I undertake to:

1. Bring my child to Mass every Sunday
2. Teach and encourage my child in becoming a full member of the Church
3. Fully participate in the Sacramental Program as my child's Parent

You can review your details and correct them if needed. Just ask at the Parish office. If your child has been baptised elsewhere we may need to inform that location of confirmation as required by Canon Law.
